Position Overview
As Registered Nurse, you advise, inform and train patients and their relatives during the move from hospital to home. You help patients feel safe using their devices, organise aftercare, support product selection and work with local doctors and health insurers on prescriptions. You care for patients at home, including weekly port changes, order aids, write visit reports and keep nursing documentation on an iPad. You also present PubliCare as an aftercare provider in new clinics and maintain contacts with clinical and outpatient partners.

Key Responsibilities
Patient Advice & Training
- Advise, inform and instruct patients and relatives during the move from hospital to home

Qualifications
Required
- Completed training as a qualified health and nursing professional
- Practical experience caring for nutrition patients
- Confidence in parenteral and enteral nutrition care
- Valid car driving licence and living in the NRW region
Nice to Have
- Experience in field service or outpatient care, but not mandatory
- Independent, well-organised work style
